Arguments

mr: Model response object returned by Surv
time: Max time
causen: Cause of interest (numeric)
type: Outcome type, "cuminc", "survival", or "rmean"
ipcw.method: "binder" or "hajek"
Gi: vector of estimated censoring probabilities
